After initial consultation, NP Housecalls nurse practitioners visit regularly with patients, not only monitoring chronic conditions, but also truly getting to know our patients. Often, all it takes is a very subtle change in an elder’s personality and/or function to indicate the onset of an episodic illness or an exacerbation of a chronic condition. Timely medical assessment and intervention from NP Housecalls may prevent hospitalization and an extended skilled nursing facility admission. NP Housecalls is determined to minimize these episodes in order to maximize quality of life for our patients and their families.
